TITLE: Service Coordinator
REPORTS TO: Ongoing Service Coordinator Supervisor, Director of Social Services and Director of Client Services
PURPOSE:
Under the supervision of the Director of Social Services, Director of Director of Client Services and the Chief Executive Officer is responsible for the delivery of Early Intervention Services. Responsibilities include but are not limited to:
RESPONSIBILITIES:
Supervision:
1. Guide families through the Early Intervention and Community Preschool Education (CPSE) process.
2. Ensure to maintain proper documentation on families and children in accordance with Federal, State, and municipal standards.
3. Act as a liaison between therapists, families and providers when necessary.
4. Collaborate with our internal resources and outside providers to secure needed services for families.
5. Ensure that approved services are provided in a timely manner.
6. Develop plans and strategies to meet the family’s needs.
7. Participate in the development, implementation, and monitoring of the Individualized Family Service Plan.
8. Maintain regular and ongoing contacts with families where necessary.
9. Build relationships with other agencies, providers and Early Intervention Official Designees and other representatives.
10. Implement program policies and procedures, which include intake, evaluation, and assessment, IFSP, referral and transition.
11. Maintain a caseload in accordance with Agency standards.
12. Attend regularly scheduled Agency and program meetings.
13. Help families promote the healthy growth and development of their child through teaching modeling and developmentally appropriate activities.
14. Demonstrate an understanding of all programs direct service options and accurate documentation.
15. Participate with multidiscipline evaluation team as needed.
16. Organize evaluation packages as required by the program standards upon receipt and forward to appropriate County personnel, if needed.
17. Perform other duties as directed or required.
Professional Requirements:
1. Must be able to speak, write and read the English language fluently.
2. Must possess other language skills such bilingual and or proficient in American Sign Language.
3. Possess the ability to make independent decisions when circumstances warrant such an action.
4. Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle personnel based on whatever maturity level they are currently functioning.
5. Must possess leadership ability and the willingness to work harmoniously with others.
6. Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures of the family.
7. Must have ability to have liaison between providers and management.
8. Must maintain the care and use of administrative supplies, equipment, etc., and maintain the personnel department in a clean, safe, and attractive manner.
9. Must possess excellent time management skills.
10. Must have the ability to maintain confidentiality with respect to clients, personnel, and any other workplace related matters.
11. Use and disclose PHI only as authorized, as necessary to carry out job duties.
12. Complete privacy and security training.
13. Must have basic computer skills such as Word, Excel, MS Access, and any other programs and possess the ability to acquire new computer skills.
Educational Requirements:
1. Must hold a minimum of an Associates degree in health care or related field.
2. Participation of general Early Intervention trainings in all Counties that are serviced by the Agency.
3. Have relevant coursework in specializing in children from birth to age 5 with disabilities.
Physical Requirements:
1. Must be able to move intermittently throughout the workday.
2. Must be able to cope with the mental and emotional stress of the position.
3. Must possess sight/hearing senses, or use of prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
4. Must function independently, have flexibility, personal integrity, and the ability to work effectively with personnel and support agencies.
5. Must be in good general health and demonstrate emotional stability.
6. May be required to lift, push, pull and move office equipment, supplies, etc. throughout the workday.
7. Must have a medical annually and be in compliance with Agency, federal, state and municipal statutes.
8. Must undergo a criminal background check and a child abuse check as required by Federal, State and Agency guidelines.
EXPERIENCE:
1. Must have a minimum of 2 years experience in Early Intervention or other health related field preferably in child development.
WORK ENVIRONMENT:
· Sits, stands, bends, lifts, and moves intermittently during working hours.
· Is subject to frequent interruptions.
· Is involved with personnel under all conditions/circumstances.
· Is willing to work beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.
· Attends and participates in continuing educational programs designed to keep you abreast of changes in your profession.
· Is subject to hostile and emotionally upset personnel.
· Maintains a liaison with other department directors/supervisors concerning personnel matters.
